On one end, you've got hypothyroidism, where the thyroid is sluggish, like a sloth on a lazy Sunday, leading to symptoms like weight gain, fatigue, and feeling cold all the time. On the flip side, hyperthyroidism kicks things into overdrive, like a caffeine-fueled cheetah, causing rapid weight loss, heart palpitations, and sweating profusely. Then there's the fun of thyroid nodules, little lumps that can either be harmless bystanders or potential troublemakers. And let's not forget about thyroid cancer, a less common but severe concern that requires prompt attention. So, whether your thyroid is taking an extended nap or doing a marathon sprint, keeping an eye on its function is vital to staying in tip-top shape.
Hypothyroidism and hyperthyroidism are two conditions that affect the thyroid gland but have opposite effects on the body:

When it's under attack from your own immune system, it can't produce enough hormones, leading to symptoms like fatigue, weight gain, sensitivity to cold, and sometimes even changes in mood or skin texture.

With the right treatment and monitoring, many people with Hashimoto's can lead perfectly normal lives. Please join me in welcoming Dr. Dimitris Tsoukalas. There’s a chronic condition that’s more prevalent than cardiovascular disease and cancer combined. Autoimmune disease is the most common chronic illness, with over 50 million people suffering from autoimmune conditions in the US. Autoimmune disease is a condition where the immune system does not recognize certain organs and tissues, so it attacks them. Hashimoto’s is the most common autoimmune disease. Dr. Tsoukala explains that Hashimoto's thyroiditis is a chronic inflammatory condition in which the immune system attacks and gradually destroys the thyroid gland. This reduces the thyroid's function, leading to hypothyroidism. Around 26 million people in the United States are suffering from Hashimoto's disease—and 9 out of 10 are women. One of the first symptoms of Hashimoto’s is low energy. This is because the hormones produced by the thyroid gland regulate metabolism and control the metabolic rate. People with Hashimoto’s also have difficulty regulating body weight because their metabolic rate is slow. Hormonal imbalance and emotional instability are other common symptoms of Hashimoto’s. All of Dr. Tsoukalas’ patients diagnosed with Hashimoto’s also have vitamin D deficiencies. More than 90% of his Hashimoto’s patients are insulin resistant. Another common problem associated with Hashimoto’s is multiple marginal micronutrient deficiencies. These deficiencies lead to mitochondrial dysfunction. Doctors are trained to look at a very broad reference range which includes people who are both healthy and unhealthy. A better way to look at your thyroid lab tests is by narrowing down the reference range to what I call "optimal" levels. You can use these optimal levels to tell if your thyroid is ACTUALLY working correctly. In addition, you'll also find that there are far more thyroid lab tests in my recommended set when compared to the TSH and maybe free T4 that your doctor might order. By looking at all of these thyroid lab tests (TSH, free t3, free t4, total t3, reverse T3, thyroglobulin antibody, and anti TPO antibody) you will get a much better picture of thyroid function.

Men and women can have an thyroid issue, but a woman is about five to eight times more likely to be Diagnosed with a thyroid condition than a man. Approximately 1 in 8 women will be affected by a thyroid condition at some point in their lives. Hypothyroidism, a slow thyroid function the most common thyroid issue, and Hyperthyroidism, a fast thyroid function, problems tend to develop slowly, often over a number of years. The standard treatment for hypothyroidism is taking daily thyroid hormone replacement medication, but medications come with side effects. Four common issues associated with the thyroid include Hashimoto's thyroiditis, Graves' disease, goiter (enlarged thyroid), and thyroid nodules.
Let's explore the thrilling world of thyroid hormones! Meet T3 and T4, the dynamic duo of metabolism. T4, aka thyroxine, is the fuel tank, storing energy with its tyrosine backbone and four iodine atoms. T3, or triiodothyronine, is the turbocharged fuel, smaller and more potent with three iodine atoms, ready to rev up our metabolic engine. While T4 may outnumber T3, it's T3 that packs the real punch, influencing body functions. Together, they ensure our bodies stay energized and running smoothly!
If you suspect thyroid issues are at play, several tests can provide valuable insights into your thyroid's function. Here are the main ones:
Blood Tests:
1. Thyroid-Stimulating Hormone (TSH) Test: This test measures the level of TSH in your blood. TSH is produced by the pituitary gland and stimulates the thyroid to produce hormones. Elevated TSH levels typically indicate an underactive thyroid (hypothyroidism), while low levels may suggest an overactive thyroid (hyperthyroidism).
2. Thyroid Hormone tests measure the levels of thyroid hormones, such as thyroxine (T4) and triiodothyronine (T3), in your blood. Low levels of free T4 and free T3 often accompany hypothyroidism, while high levels may indicate hyperthyroidism.
3. Thyroid Antibody tests check for antibodies that attack the thyroid gland, which can indicate autoimmune thyroid disorders like Hashimoto's thyroiditis or Graves' disease.
Types of thyroid antibodies that are associated with autoimmune thyroid disorders:
1. MAK (TPO): This likely refers to Thyroid Peroxidase Antibodies (TPO antibodies), which are antibodies that attack an enzyme called thyroid peroxidase. Elevated levels of TPO antibodies are commonly found in autoimmune thyroid conditions such as Hashimoto's thyroiditis.
2. TAK (TAab): This abbreviation is less commonly used but could potentially refer to Thyroid Antibody Kinetics. This term may describe the dynamics or behavior of thyroid antibodies in the body over time.
3. TRAK (TSI): This likely refers to Thyrotropin Receptor Antibodies (TRAb), which are antibodies that target the thyroid-stimulating hormone (TSH) receptor. Elevated levels of TRAb are associated with Graves' disease, an autoimmune condition characterized by an overactive thyroid gland.
Other Tests:
1. Thyroid Ultrasound: This imaging test uses sound waves to create images of the thyroid gland. It can help detect thyroid nodules or abnormalities in gland size and structure.
2. Thyroid Scan: In this test, you're given a small amount of radioactive iodine, which is taken up by the thyroid gland. A special camera then detects the radioactive iodine to assess thyroid function and identify any nodules or areas of abnormal activity.
3. Fine-Needle Aspiration (FNA) Biopsy: If thyroid nodules are detected, your doctor may recommend an FNA biopsy to determine whether they're cancerous or benign. A thin needle extracts cells from the nodule for examination under a microscope during the procedure.
Many doctors rely solely on TSH levels to diagnose thyroid disorders, often overlooking the importance of testing Free T3 and Free T4 levels.
Here's the deal: TSH is like an orchestra conductor, signaling the thyroid gland to produce thyroid hormones (T3 and T4) in the right amounts. However, just like any conductor, TSH isn't infallible. It can give misleading signals, especially if there's a glitch in the system.
That's where Free T3 and Free T4 come in. These hormones are the real players in the thyroid game, regulating metabolism, energy levels, and overall well-being. Testing Free T3 and Free T4 directly measures the actual thyroid hormone levels circulating in your body, providing a more accurate assessment of thyroid function.
But here's the rub: Even if your TSH levels fall within the "normal" range, your Free T3 and Free T4 levels could still be off-kilter. It's like having a perfectly functioning thermostat (TSH) in a house with faulty heating (low T3/T4 levels) – everything seems fine on the surface, but you're still shivering in the cold.
This disconnect between TSH and actual thyroid hormone levels can lead to countless individuals slipping through the cracks and experiencing symptoms of thyroid dysfunction despite "normal" TSH results. That's why advocating for comprehensive thyroid testing, including Free T3 and Free T4, is crucial for accurate diagnosis and proper treatment.
So, if you suspect thyroid issues are at play and your symptoms don't align with your TSH results, don't hesitate to speak up and request further testing. After all, your health is too important to leave to chance!
